Step 1:  Contact us via our contact form.

Step 2:   Fill out the Medical History forms on our website, they can be found on the medical forms page, simply pass your mouse over the document links on the medical forms page, right mouse click and save them locally on your desktop to be opened, print out, filled out and then fax them back to us. Call us after faxing them back to us.

Step 3:   Once we review the completed Medical History form, we will schedule you to get the required bloodwork / comprehensive medical evaluation local to you - your bloodwork and physical exam will be done in a single convenient appointment. We have a national network of labs and medical doctors, we will locate one close to you for simplicity and convenience. If you already have bloodwork and it is less than a year old we can use it if it has all of the blood tests we require, download the list of required bloodwork from our medical forms page to compare, if you are missing any tests we can have you make them up at a lab. :: Pre-Placement Physical Exam ::

Step 4:   Once we receive your bloodwork / medical evaluation about a week later, our doctor will review them, call you and discuss your results and then help you co-create an HRT program that it most suitable to you and your personal needs.

Step 5:  Our doctor will contact you and have virtual Face-Face Consultation, to establish a Doctor-Patient-Relationship and discuss all dosages, side effects, and results you should expect.
